Directions for PDF Forms

Entry forms can be completed on your internet browser; however, you will not be able to save the form as a file to email, unless you have the Adobe Writer.

If you only have access to the Adobe Reader do the following:

• Type the information into the boxes
• Print out the completed typed form
• Fax the completed form with appropriate signatures, to the championship convenor

IMPORTANT: If you download a form and the page is blank and/or generates errors, you probably need to upgrade your version of Adobe Acrobat ® reader
